Анотація: Стаття присвячена комплексному економічному аналізу формування, інституційного становлення та структурної трансформації сектору охорони здоров’я України у період 1991–2025 років. Дослідження ґрунтується на систем-ному макроекономічному підході та охоплює еволюцію фінансової моделі галузі від радянської ресурсно-утримувальної системи до сучасної моделі стратегічних за-купівель медичних послуг через механізм єдиного платника. Особливу увагу приді-лено оцінці ефективності впровадження Програми медичних гарантій, автономі-зації закладів охорони здоров’я та зміні принципів алокації бюджетних коштів.
У роботі проаналізовано динаміку державних видатків, структуру джерел фінан-сування, рівень кишенькових витрат домогосподарств, а також розвиток прива-тного медичного сектору, фармацевтичного ринку та сегмента медичних тех-нологій. Встановлено, що попри суттєве номінальне зростання бюджетного фінансування, система зберігає структурні дисбаланси, зумовлені історично ви-сокою часткою прямих витрат населення на медичні послуги та лікарські засоби. Окремо досліджено макроекономічні наслідки повномасштабної війни, зокрема руй-нування інфраструктури, міграцію трудових ресурсів, зміну географії попиту та трансформацію інвестиційних потоків.
Емпіричну базу становлять офіційні статистичні дані державних органів, міжна-родних організацій та галузевих аналітичних звітів. Проведено структурно-дина-мічний аналіз розвитку фармацевтичного ринку, який демонструє зростання у грошовому вимірі на тлі скорочення натуральних обсягів споживання, що свідчить про інфляційний характер розширення ринку. Досліджено роль приватного капі-талу, добровільного медичного страхування та прямих іноземних інвестицій у за-безпеченні фінансової стійкості галузі.
У результаті зроблено висновок, що сектор охорони здоров’я України перебуває у фазі керованої трансформації, поєднуючи ринкові механізми фінансування, цифро-візацію та інституційні реформи з адаптацією до воєнних макроекономічних шоків. Подальший розвиток галузі визначатиметься незворотністю реформ, залучен-ням довгострокового інвестиційного капіталу та інтеграцією у європейський ме-дико-економічний простір.
Abstract: This article provides an integrated economic assessment of the formation and long-term evolution of Ukraine’s health care sector from 1991 to 2025. The study applies a macroeconomic and institutional framework to examine the transition from the inherited centralized, input-based financing model to a system grounded in strategic purchasing and a single-payer mechanism. Particular emphasis is placed on the financial transformation initiated in the late 2010s, including the introduction of performance-based funding instruments, hospital autonomy, and the reorientation of public spending toward service outputs rather than infrastructure maintenance.
The research investigates trends in public health expenditures, the composition of funding sources, and the persistent role of out-of-pocket payments. It also explores the expansion of private medical providers, the development of the pharmaceutical industry, and the growing importance of medical technologies within the broader sectoral ecosystem. Despite substantial nominal growth in government allocations, structural imbalances remain, largely due to the historically significant burden placed on households in financing medicines and specialized services.
A separate analytical dimension addresses the economic consequences of the full-scale war, including infrastructure destruction, workforce displacement, regional redistribution of demand, and shifts in investment patterns. Using official statistics and verified industry data, the paper identifies a divergence between monetary growth and physical consumption volumes in the pharmaceutical market, indicating inflation-driven expansion amid demographic contraction and currency pressures.
The findings suggest that Ukraine’s health care system is undergoing a complex but controlled structural transformation. Market-oriented financing tools, digital governance platforms, and enhanced transparency mechanisms have strengthened resilience under extreme macroeconomic stress. The future trajectory of the sector will depend on the continuity of reforms, the mobilization of long-term domestic and foreign investment, and deeper alignment with European standards in health governance and financing.
